How to Register

• Print and complete the registration form, and sign the informed consent.
• If you are a new player to Spartans, attach a copy of proof of age (birth certificate, passport, citizenship card)
• Attach the required fees. Please make cheques payable to Spartans Football
• Mail registration form, signed informed consent and fees to:
